  • Termination of ALL prior marriages (divorce decrees, annulments, or death certificates for both petitioner and beneficiary)

Evidence of Bona Fide Marriage
  • Joint bank account statements
  • Joint lease or mortgage documents
  • Joint utility bills (electric, gas, internet)
  • Joint insurance policies (health, auto, life)
  • Birth certificates of children born to the marriage
  • Photographs together at different times and places
  • Affidavits from friends/family attesting to the relationship

  • Birth certificate of the child (showing both parents' names)
  • Marriage certificate of child's parents (if claiming child born in wedlock)

  • Birth certificate of the stepchild
  • Marriage certificate of the petitioner and the child's birth parent
  • Proof the marriage occurred before the child turned 18
  • Termination of any prior marriages (divorce decrees or death certificates)
Father for Child (Born Out of Wedlock)
  • Birth certificate of the child (showing father's name)
  • Evidence of legitimation or bona fide parent-child relationship
  • Affidavit of parentage or DNA test results (if father not on birth certificate)
  • Evidence of emotional or financial support
Adoption-Based
  • Adoption decree or certificate
  • Proof the adoption was finalized before the child turned 16 (or 18 if sibling exception applies)
  • Evidence of 2-year legal custody AND 2-year joint residence with the adoptive parent

Filing fee ($535 — check USCIS for current amount)
Proof of petitioner's U.S. citizenship or LPR status:
• U.S. birth certificate, U.S. passport, Certificate of Naturalization (N-550/N-570), or Certificate of Citizenship (N-560/N-561)
• OR copy of Green Card (front and back)
Petitioner's government-issued photo ID
Beneficiary's passport bio page (copy)
Beneficiary's birth certificate with certified English translation
2 passport-style photographs of the beneficiary
Form G-1145 (E-Notification of Acceptance, optional)
All foreign-language documents must be accompanied by a certified English translation.
